Case Study

Mary is an 87 year old woman who has been living alone in her home since her husband died 10 years ago.  She had a fall and broke her wrist and is no longer able to safely care for herself at home.  She is being transferred to a care home from the hospital.  The LPN must ensure that pertinent information is obtained to ensure that Mary is given the best care possible.  The doctor provides the current list of medications as well as a summary of her medical history.

Mary’s current conditions include hypertension, Chronic Obstructive Pulmonary Disease (emphysema) from years of smoking,osteoarthritisand shehaspatches of eczemaon her lower legs and arms.  While being treated for her fractured wrist, she received narcotics that caused her to be constipated and in addition she was diagnosed withAlzheimer’s disease.
