Strategic Health Services Management

A structured strategic management is very necessary as an approach needed in tackling revolutionary changes in the healthcare system. Health care organizations have today universally embraced the strategic management perspectives developed in the business and heath sector. The health care systems have developed strategies and management systems that are unique in their own ways. According to Swayne, Duncan and Ginter, P. (2013), health care leaders have realized that strategic planning, thinking, and managing strategic momentum are important for coping with the dynamic nature of the health care industry. Strategic management has become the single most effective manifestation of effective and reliable leadership in the health care organizations. Strategic management provides a guiding framework for the management of all kinds of contemporary health organizations.

Over the past few decades, strategic management concepts have been widely applied in the healthcare industry. Swayne, Duncan, and Ginter (2013) notes that the concept of strategic management has continuously been embraced or adopted by healthcare organizations as a core part of leadership and as one way of anticipating and coping with different external forces. The key purpose for strategic management in health care system is to ensure that there is a fit between the external and internal environment of an organization. To ensure survival and compatibility, health care organizations should have the ability to comprehend the significant shifts in environmental conditions and be able to equip their organizations to be able to succeed in the new environments. So many health care organizations are set up every passing day and they are formed based on new technologies and new forms of operation that if the old health organizations do not have the strategies to analyze their operations, they can easily be edged out of the market. This calls for constant and persistent reforms in relation to the changing external environment (Swayne, Duncan, and Ginter, 2013).

Strategic Management in health care can be considered in a number of ways in relation to how different factors affect the American citizens. The first strategic management in this aspect is the cost management. The rising costs for the American patients in the healthcare industry has motivated the providers to make efforts in managing such costs, while at the same time offering quality care at affordable prices. According to Swayne, Duncan, and Ginter, (2013), one major step taken by the providers in reducing costs is analyzing and establishing the inefficiencies associated with the operations of heath care organizations. The next point is the aspect of information management, which is a key factor in the management of both the mission and the costs incurred by healthcare providers. An increase in the availability and the quality of information systems, like computer software, hardware, and networks, can boost the knowledge and operations of the providers and at the same time improve the quality of healthcare provided to the patients. The availability of computers will ensure that all the health records of the patients are safely stored for faster accessibility and also used in the future to obtain any important information (Swayne, Duncan, and Ginter, 2013).

The next strategic management is system management, which when properly managed can provide both quality patient care and high technology which may be difficult for single facilities to access single handedly. For example, the health care system of the Indiana University is formulated in such a way that all patients can access whatever resource they need across the state. It has a central system office that is able to coordinate all the services in its member hospitals in various cities across the state. Gathering such resources and information enables small business owners, their employees, and even patients to access a large volume of information within a very short time. The other aspect is the patient care management, which may include diagnosis, laboratory testing, examination, treatments, prescriptions, and surgery. These services can be provided in state-of-the-art mobile care facilities, hospitals, or even in a physician’s office, by the providers. According to Swayne, Duncan, and Ginter, (2013), the major challenge facing the health care industry is the manner of managing the different types of patients. The challenge comes in on how to serve all the customers at low costs and at the same time maintain high quality services.

In any hospital environment, there are strategic changes that often take place and need to be put into consideration. For a health organization to continue performing, there is need to adopt new polices in order to remain relevant in its operations. The organization can for example make use of a research focusing on the future trends to elucidate whether a certain strategy change would help in the achievement of goals and objectives. However, certain strategies may promote the accomplishment of some set of goals but at the same time work against another goal. This implies that one strategy can at times conflict another, thereby calling for the need of health providers to understand strategic co-ordination. According to Swayne, Duncan, and Ginter, (2013), most of the organizational and environmental characteristics are negatively associated with the operational efficiency or market share in the health care environment. Only for-profit status and stringent Medicare payments correlate with proper financial performance. This implies that urban hospitals lose market shares in wealthier, competitive, and highly populated communities. The urban hospitals have on the other hand strived to overcome this challenge by diversing their operations across many regions within the state so as to expand on their volume of clients (Swayne, Duncan, and Ginter, 2013).

SOSI

The SOSI (Statement of Strategic Intent) for C. W. Williams Health Center is to promote a better future, in terms of health for the community; by continuously giving the best, reachable healthcare; with respect, compassion, and pride following the vision of the organization’s founder-Dr. Charles Warren Williams. The strategic statement also indicates that C. W. Williams need to advance into the 21st Century through promoting a brighter and healthier future of the black community. The health center’s Chief Executive Officer, Michelle Marrs, with her 20 years experience in health care management and efficient delivery of services was the major force behind the center’s financial improvement and the efficient provision of health care. Her efforts entailed optimizing and rationalizing operation efficiencies, increasing payment rates to the heath center by initiating insurance systems, starting partnerships with several regional providers of healthcare services aimed at improving the organization’s network, and strengthening the links with private providers of community healthcare services (Swayne, Duncan, and Ginter, 2013).

The chief executive had the objective of providing health care services to individuals who were medically underserved majorly in the rural areas since half of the individuals who were medically underserved lived in the rural areas. The other half that were medically undeserved also lived in economically depressed locations in the inner cities, where they lived in poverty and lacked services such as health insurance. Other individuals also had serious problems of homelessness, substance abuse, and AIDS. In order to serve all the citizens across the state, the chief executive formulated strategies that would ensure that health services is accessible to all individuals both to those who lived in the rural areas and also the other individuals who lived in the poverty stricken slums in the cities. One of the strategies was the initiation of the managed care plans, which enrolled over 2 million beneficiaries of the Medicaid program. According to Swayne, Duncan, and Ginter, (2013), low income Americans and the Medicaid beneficiaries had high rates of disabilities and common illnesses than others. This meant that they accumulated comparatively higher costs of medical cover and medical care. However, they were faced with certain challenges such as the rural nature of the Southern states, which made such states not become attractive to most managed care organizations and the insurers that provided fee-for-service to hospitals and the physicians (Swayne, Duncan, and Ginter, 2013).

The strengthening of connections with Tufts Medical School, in addition to creating plans and programs to develop several community-based health centers helped in bringing healthcare services nearer to patients. Raising finances is the immediate step into realizing a brighter future for C. W. Williams and Tufts Medical School relationship. This is a vital step given that the history of losses in finances is connected to the decline in the existence of the health center. The idea of raising finances was a major step in the operations of C. W. Williams health centre because it offered the opportunity to adequately serve the medically underserved individuals in the rural areas by making all necessary facilities accessible to the citizens (Swayne, Duncan, and Ginter, 2013).

Michelle had served in the health care sector for a very long time but she admitted facing a number of challenges due to the rapidly changing health care environment that required new means of operation every passing day. The managed care changes thus forced the health care centre to align themselves to one of the two hospitals. They were forced to stake such a step despite not willing to take away the patients’ choice due to the managed care changes. However, in order to achieve such a mission, the chief executive noted that she needed more help from internal operations before proceeding to the external interactions. She further created the position for the director of finance and at the same time striving to buy another location in order to serve its patients in a better way. The management however argued that buying another facility would seriously stretch the organization financially but on the other hand argued that finding a new location for the facility was a perfect idea since it would give a variety of patients the opportunity to access all the medical services of their choice (Swayne, Duncan, and Ginter, 2013).

APPENDIX

Stakeholder Analysis

It is vital for the board of C.W. Williams to conduct a proper stakeholder analysis so as to identify the crucial stakeholders who are capable of impacting the organization’s objectives and goals. Stakeholders are charged with the responsibility of developing and implementing different tactics and strategies based on the existing health service environment. The stakeholders should be a team of dedicated individuals who are able to transform their activities and operations according to the standards set by the dynamic global medical environment. Major stakeholders of C. W. Williams are listed below according to their significance.

In order to identify the influence these stakeholders have on the operations of C. W. Williams Health Care centre, it is important to consider each stakeholder category on a matrix based analysis. The following shows how the stakeholders have been classified:

MEETING NEEDS

Keep contented in order to meet their needs MAJOR PLAYERS

Influenced actively and closely engaged.

LEAST IMPORTANT

Monitor using minimum effort SHOW CONSIDERATION

Keep them informed

Power

High

Interest

Low

Low

High

Patients: In every organization, patients posses the highest power, but with lower levels of interest in the management of the organization (this is in reference to C.W. Williams Health Center). With this in mind, the patients’ needs ought to be met by keeping them satisfied. According to Swayne, Duncan, and Ginter, (2013), patients are the main reason for the establishment of any health facility and their interest should be paramount to the operations of any health facility. Health organizations that ignore the needs of their patients are not likely to succeed since the patients would be moving out one after the other and eventually leaving them with no patient to serve. In order to succeed, the organization should further carry out periodic surveys on the patients to sample all their desired interests and then look into the best services to offer. C. W. Williams Health Center should move into the rural areas where several individuals who are medical underserved live in order to offer proper and timely services to them. The organization has strived to acquire new facilities and medications that are able to handle serious diseases like HIV/AIDS, which is a major disease affecting the individuals living in the rural areas and the slums found in urban areas. Swayne, Duncan, and Ginter, (2013) note that the services offered to the patients should be one that is dynamic and universally acceptable.

Physicians: The physicians working at C.W. Williams Health Center are categorized under high power and high interest. Therefore, this group are the major players in the industry, thus should be handled accordingly. These individuals are at the core of operations at every health facility and they greatly influence the success of the health organization. They are at a close interaction with the patients and directly offer treatments to the patients. The organization should therefore ensure that its physicians are a team of experienced and competent individuals who will be able to offer the right kind of treatment for the right disease. Incompetent and inexperienced physicians often lead to patients losing their lives aimlessly because of poor treatments. This would give a bad reputation to the health organization and for that matter not be able to achieve its goals and objectives (Swayne, Duncan, and Ginter, 2013).

Employees/staff: The case of C.W. Williams Health Center’s staff and employees are also categorized under the important players. These two groups should be engaged with keen interest in the management of the organization. They also play a major role in the operations of the organization in the sense that how they treat and handle the patients determines the reputation of the health care center. C. W. Williams Health care center should be able to employ several employees across the state but most notably in the rural areas where the underserved individuals live and where their services are majorly required. The employees and the staff should be a team that is very friendly to the patients, punctual at the work place, and above all dedicated to the achievement of the organization’s goals and objectives. A staff that is unfriendly or rude to the patients would send a very bad image to the public and for that matter may drive away the patients that are in high need of the health services. This would otherwise contradict the organization’s mission and objective of equal service to all. In its quest to adopt new forms of technology in keeping the health records, the organization should be in a position to employ individuals who are able to handle computers to enable them keep all the health records for the patients. According to Swayne, Duncan, and Ginter, (2013), this will not only make the health records accessible for all individuals but will also offer faster delivery of health services.

Board of directors: The board is also a significant player in the stakeholder overview of the company. They have high interest and power in the management of the organization. It is vital for C.W. Williams Health Center to engage the board directly and actively while making decisions on the hospital’s operations. The board of directors should have the interest of the organization at heart by ensuring that all the operations and priorities of the company are rightfully placed. The organization should give the board of directors the power to carry out a periodic strategic audit and analysis of the activities of all employees and other stakeholders to ensure efficiency of the organization. However, the board should be comprised of individuals who have good reputation and integrity to ensure that the organization achieves its goals and objectives (Swayne, Duncan, and Ginter, 2013).

Charlotte Memorial Hospital: Charlotte is categorized under high power in the stakeholder analysis of C.W. Williams Health Center. The hospital was mandated to admit patients who were required to be hospitalized in C.W. Williams. Due to the little space in the health center, more space was needed (and it was available at Charlotte Memorial). There need to be a very close relationship based on mutual trust and respect between C. W. Williams and Charlotte Memorial Hospital so that the patients getting admitted to the hospital get quality treatment based on the mission and vision of C. W. Williams. This commitment was observed in the Chief executive officer when she said that patients are at will to choose the hospital where they receive their treatment.

Federal Government: The federal government possesses high power and low interest in the affairs of C.W. Williams Health Center management and operations. The federal government normally set up rules and regulations that directly or indirectly affect the operations of any health organization. The federal government can also dictate the kind of areas that the health facilities should be built. It can also set up rules that will discourage the citizens from using the facilities of the organization or set up rules that may lead to the collapse of the organization altogether. The federal government also determines whether or not to offer grants for the operations of the hospital. If the federal government decides not to give or reduce the amount of money meant to be a grant for the company, its mission of expanding operations to the rural areas where there are many individuals who are medically underserved would be highly affected. However, it is generally healthy for every organization in any state to observe the existing rules and regulations for purposes of developing a positive public image and also to avoid rubbing shoulders with the federal government. Therefore, the organization need to meet the legal requirement and needs of the government to ensure they are satisfied; failure to which the operations of the organization can be highly compromised (Swayne, Duncan, and Ginter, 2013).

Medicaid consulting groups: The consulting groups possess high power and low interest in terms of stakeholder significance and participation in the operations and management of C.W. Williams Health Center. These are groups that are mandated with the responsibility of offering expertise direction to the organization and devising strategic policies that would ensure that the organization operates smoothly and profitably in the health sector that is challenging and competitive. The Medicaid Consulting group is mandated with the responsibility of carrying out market research and market analysis then coming up with policies that would ensure that the organization achieves its main mission of reaching out to the medically underserved, majorly in the rural areas. The Medicaid Consulting groups should also devise ways and means to ensure that the organization adopts means of operations that are cost effective and at the same time viable in respect to the ever changing medical environment. With the above specs in stakeholder categorization, the health center should ensure that the needs of Medicaid Consulting personnel are sufficiently met (Swayne, Duncan, and Ginter, 2013).

Hospital affiliation: The hospital affiliations of C.W. Williams Health Center are categorized under high power and low interest group. This is because they are concerned with the management, treatment, referrals, and other major activities of the health center. This is a group of people that have the operations of the organization at heart and for that matter highly influence the success of the organization. They can influence the public perception of the organization and how the organization relates with other stakeholders and other agencies that are at a higher position and ability to offer funds to C. W. Health Centre. Because of this, their needs and interests should be handled considering the power they wield in the activities and management of the C.W. Williams.

Non-physician providers: This group is categorized under low power and high interest stakeholders. As much as they may not offer direct treatments to the patients, they have a great impact on the operations and success of the organization in the sense that they form a link between the health center and the external environment. This is very vital given that the Chief executive had stated that her main mission was to set up strategies that would promote the organization’s relationship with the external environment. The organization can not advance its operations externally if there is no channel to link its activities; in this case it would be the role of the non-physician providers to accomplish such a duty. Therefore, C.W. Williams should ensure that they are informed while at the same time showing them greater considerations (Swayne, Duncan, and Ginter, 2013).

Pfizer: Like the Medicaid Consulting group, Pfizer as a major provider of drugs and other important medical aids have high power and low interest in the management and operations of C. W. Williams Health Center. This is another major stakeholder of the C. W. Williams because of their role of distributing drugs to the hospital. Their failure to deliver drugs in time would mean that the patients would not be treated in time, which would mean putting danger on the lives of the patients. The hospital should for that matter ensure that Pfizer becomes a very responsible and reliable team that can be trusted with the delivery of drugs at any time of need. The organization should also ensure that Pfizer produces drugs that are effective in treating the patients and at the same time have long expiry dates so that the drugs can stay long in the hospital shelves without expiring. Since the organization has the potential of benefiting the organization in terms of realizing high profitability; C. W. Williams need to ensure that their needs are catered for (Swayne, Duncan, and Ginter, 2013).

Trend Analysis for C. W. Williams Health Center

There a number of possible events that can emerge with reference to both internal and external situations in C. W. Williams Health Center. Some of these possible events are:

Partnering of Area Hospitals for form HMO Shift from FFS to Managed Care Environment

Increasing the number of community health centers

Increasing need for medical technology

Reduction in government allocation to grant money for C. W. Williams.

The listed trends are categorized and analyzed based on the impact they have on the organization in relation to their profitability on the overall performance of the organizational activities as shown below:

Service Area Competitor Analysis for C.W. Williams Health Center

Service area competitor analysis enables organizations to understand and analyze the significant patterns and factors influencing competition based on the location of the company or the business it is involved. Porter’s 5 forces technique is one of the major strategic tools that can be applied in the assessment and evaluation of service area outlook in relation to the competition faced by the company. Porter’s technique evaluates the industry in which the organization is operating and the competitive rendition using the following main forces:

The suppliers bargaining power

The buyers bargaining power

Threat emerging from substitute services and products

Threats emerging from new entrants into the business

Existing rivalry among or between the established competitors

Using the above forces, C. W. Williams Health Center’ service area competitor analysis can be expressed as follows:

The suppliers bargaining power

The suppliers have a low to moderate bargaining power in the healthcare provision industry targeting the specific community (black). The industry is marked with the presence of well-established suppliers who are capable of providing the same products and services at significantly lower costs. The fact that there exist so many suppliers in the medical industry who are able to offer high quality services and health facilities increases competition among the suppliers. It becomes a situation of many suppliers fighting for a few health providers in the medical industry. The result would be that the suppliers would produce high quality drugs and at the same time supply them at lower prices, which otherwise works to the advantage of C. W. Williams since this gives it the opportunity to serve its patients with high quality drugs. The fact that it is able to purchase the drugs at a lower price will in turn lower its cost of operation and thus give it the opportunity to offer reasonable remuneration to its staff and employees. This will in turn give the employees the motivation to work even harder (Swayne, Duncan, and Ginter, 2013).

The buyers bargaining power

The buyers have a high to moderate chances of bargaining power in the healthcare provision industry. In the locality where C. W. Williams is operating, different institutions and health care centers exist; providing similar services and products to the targeted patients hence the increase in the opportunities of bargaining power to the buyers of the services and products. There are so many hospitals in the region but the chief executive stated that every patient is entitled to choose the hospital to receive medical attention. The loyal customers to W. W. Williams will always stick to their services given the quality services they offer and their desire to serve individuals who are not so privileged in the society. However, when it comes to buying drugs, the buyers will always move to the health providers that offer a variety of drugs at lower prices. This calls for the need of C. W. Williams to co-ordinate with manufactures that produce high quality drugs but at comparatively low costs in order to gain a competitive advantage in its environment of operation (Swayne, Duncan, and Ginter, 2013).

Threat emerging from substitute services and products

Threat emerging from substitute products and services in the healthcare provision to the community is considerably high. The explanation behind this occurrence is managed care provision, which has very high influence on service delivery to marginalized communities and other healthcare institutions. In addition to this, various organizations and community based institutions in the United States of America have developed their own programs assisting the employees. This includes companies that have opened hospitals where its patients can be treated at highly subsided rates and also selling high quality drugs at lower prices. This gives C. W. Williams a competitive disadvantage as the individuals who would have been treated at their health centre get fair treatments elsewhere. The Federal government also has the tendency to open public health centers where most services are free or the drugs are sold at very low costs. Due to the fact that the government operates under economies of scale, it becomes very difficult for the organization to effectively compete in the provision of health services (Swayne, Duncan, and Ginter, 2013). This leaves a negative effect on the institutions charged with the responsibility of giving healthcare to the marginalized communities in any location. The Federal government may also fail on its duty of making the rural areas accessible for the establishment of new health centers. This may also offer a great challenge to C. W. Williams in its objective of reaching out to the marginalized groups.

Threats emerging from new entrants into the business

The threat emerging from new ventures into the industry is low. This follows the fact that the creation and maintenance of a completely new equipment or hospital facility for the provision of specialized healthcare to specific cases. For example, in C. W. Williams’ case, requires heavy investment in terms of effort and capital. Thus, new ventures are left with no option but to consider spending time and resources in their bid to enter and succeed in the industry. It is believed that such efforts are not very attractive to several small businesses (Swayne, Duncan, and Ginter, 2013). C. W. Williams also has the objective of opening up its operations to the interior parts of the village where there are very limited development and accessibility. This makes it very unattractive for other entrants that are only attracted to regions that are already established. Other entrants also begin their operations in such areas but find it quite challenging prompting them to give up and relent on the way. The Federal government also provides grants and benefits to organizations that venture in regions that pose many challenges such as the rural areas that have poor infrastructure, poor technology, and few industries. This gives C. W. Williams a competitive advantage over other entrants while at the same time forming barriers for any new entrant. Therefore, C. W. Williams Health Center is better placed at realizing less competition from new operators in healthcare provision in the area.

Existing rivalry among or between the established competitors

There is a high competition existing among the providers of healthcare services related to C. W. William’s case. Different specialized community healthcare providers exist, thus elevating the competition in the industry. Currently, healthcare institutions including hospitals are undergoing difficulties in the reorganization and restructuring endeavors aimed at ensuring their competitiveness and survival in the industry. As much C. W. Williams has some level of competitive advantage in the rural areas where it established its operations, the competition it faces from large and established health care institutions can never be underestimated. In addition, the rivalry existing between the large and established healthcare institutions also directly affects the operations of the organization since this would mean unhealthy competition within the market, which is not favorable for the operations of C. W. Williams.

Effective Treatment Interventions with Family

Introduction

Family treatment interventions are vital as they have been designed to deal with specific problems affecting people’s functioning and health. It is common for families to go through rough times, significant transitions, behavioral and mental problems hence the need to employ effective treatment interventions. The type of therapy and technique used to treat families depends on the target population and the specific issue affecting the individuals. This literature review aims to analyze and interpret family-based interventions used on various individuals within the family set up when they are experiencing health difficulties. This research could not be timelier; it comes when little research has been conducted on the effectiveness of various interventions used to treat families. The following review confirms that practices including functional family therapy (FFT), goal-oriented parental interventions, family-focused alternatives ,community-based alternatives to incarceration, family interventions for schizophrenia, and the psychoses and family-based management interventions for children with obesity improve have proved effective in improving family well-being.

Community-based and family-focused alternatives to incarceration

Ryon, Early, and Kosloski evaluated community-based and institutional placements intervention programs for delinquent youths in 2017 intending to examine the impact of a family-focused approach to deal with juvenile offenders. The authors assessed Parenting with Love and Limits (PLL), a family-focused and community-based approach piloted in Florida to treat juvenile offenders with the aim of diverting youth away from residential facilities. Ryon, Early, and Kosloski discovered that the youth that had gone through the PLL program had averagely low recidivism rates. The article opined that community-based interventions attained lower rates of a felony conviction, reconviction, and subsequent placement in a justice system after one year of completing the program. The article concluded that community-based programs that incorporate both group and family-focused individuals are viable and effective alternatives to institutional placements are more restrictive for delinquent youths.

Functional Family Therapy (FFT).

Sexton (2010) found functional family therapy (FFT) to be an efficient and successful family-based treatment program in treating various problems that affect young people, including mental health problems, drug and substance abuse, conduct disorder, truancy, and other related problems. Noteworthy Sexton found that Functional Family Therapy treats families within various multicultural, multiethnic, and geographical contexts. Sexton discovered that the FFT program benefited more than 40,000 youths and families annually following its implementation in more than 300 communities across Europe and the United States over the past few decades. Moreover, Sexton noted that the program’s delivery and implementation in communities is based on five theoretical principles. He also noted that FFT employs technologies to enhance its outcomes, including the use of measurement feedback systems, a systematic measurement system, and a web-based quality improvement system. The article notes that since the FFT family-based treatment program is manual driven, these tools act as an accompaniment to the supervision and treatment manual protocol.

Parent Engagement and Family Reunification for Children in out-of-home Care

Lack of parental engagement remains the greatest challenge for family re-unitement after temporary child placement in out-of-home care services. Maltais, Cyr, Parent, and Pascuzzo (2019) conducted meta-analyses on studies for 2996 families to identify the most effective practices that can be used to promote family reunification and parental engagement. The authors examined the efficacy of goal-oriented parental engagement interventions and compared them to a group whose parents had standard services. The article revealed that parents that received goal-oriented engagement strategies were more and had higher chances of a smooth reunification with children receiving care away from home than parents that received standard services. The authors analyzed six interventions in the analyses, including training of child care staff, strategies promoting access to intervention, type of clinical modality, sources of motivation, focusing on the relationship between child care staff and parents and a number of clinical strategies. The authors came to the conclusion that results that family-focused interventions are effective in promoting parental engagement.

Family engagement for schizophrenia and the psychoses

McFarlane (2016) reviews the family interventions for Schizophrenia and Psychoses, treatment programs that came into being 40 years ago. As opposed to concentrating on the traditional goal of decreasing emotion to prevent relapse, McFarlane focuses on the social and role functioning of the family well being. The article refutes claims that family pathology leads to deterioration and relapse and affirms that family engagement in psychoeducation is an effective alternative. McFarlane noted that family engagement complements interventions provided by clinicians with specialized coping and interaction skills that counter the neurologic deficits inherent to the particular disorder. McFarlane concluded that family interventions indicate a 50-60% relapse rate reduction compared to usual treatment. He found that a combination of the prodromal psychosis alongside other evidence-based interventions is yielding promising results that include significant return to normalcy and total avoidance of psychosis altogether.

Parental Involvement in Treating Childhood Obesity

Faith-based interventions have solutions that are commonly employed when treating childhood obesity. Chai, Collins, May, Brain, See, and Burrows (2019) review the effectiveness of interventions for managing weight that are family-based in children with obesity. The article reveals that when it comes to treating childhood obesity, direct parental involvement improves the outcomes. The authors carried out systematic reviews on family-based interventions in children below 18 years, categorized their weight as obese or overweight. The article revealed that 13 in 14 of the total reviews proved successful in improving weight-related behavior in children. Chai, Collins, May, Brain, See, and Burrows concluded that family-based interventions that target parents either single-handedly or with their child have proved useful in managing child weight.

Conclusion

Practices including functional family therapy (FFT ), goal-oriented parental interventions, community-based and family-oriented alternatives to incarceration, Family interventions for schizophrenia, and psychoses and family-based weight running interventions for overweight and obesity children have proved effective in improving family well-being. There is a need to conduct further research to complement the lack of substantial evidence, particularly regarding the emerging interventions for parents in addressing obesity in children. Health practitioners must collaborate with change agents and focus on being role models for children to emulate when managing their weight. Future research should explore parent-child interventions and include a variety of populations.

Effects of New Media

(Author’s name)

(Institutional Affiliation)

One can view how the society gets its information in three main ways. The first way is that individuals or the society are gullible and will almost listen to, read or watch anything. The second is that most individuals need an intermediary who is informed to show and tell them what is right, meaningful and important, the third is that individuals are smart and knowledgeable, and given the means, they understand and find out things for themselves and find their own truth version (Rayuso, 2011). This paper will mainly focus in the second way to view how people are informed, the fact that individuals need an intermediary that is informed to show and tell them what is meaningful and right. This informed intermediary in this case is the media, which holds all the information the public needs. The paper, therefore, will examine ways in which the media, playing the part of an informed intermediary, affects and influences the American culture by looking at the responsibility it has to the people and the influences it has on politics and journalism.

In the past fifty or more years the effects and influence of the media on the society have grown exceedingly chiefly with the progress in science and technology. First, there were telegraphs and then radios, magazines, newspapers, TV and currently the internet. Today’s society depends on communication and information to move towards the right direction and do everyday tasks like entertainment, work, education, health care, travelling, personal relationships and any other activity. The average American person wakes up each day and checks information and news on the TV and newspapers before going to work where he or she makes several phone calls in a day and makes contact with their family when it is possible and basically, leaves his or her life based on the information and knowledge derived from the media (Rayuso, 2011).

The mass media have essential tasks and roles in the modern society as the main communication channel. The populations depend on the media as the central source of information and the foundation on which they base their opinions and election and voting decisions. As the cultural selection theory argues, any choice of messages and information in the mass media will, therefore, have a momentous consequence on the rest of the society. There is an increasingly interesting situation in the current society: on one side, the media have significant influence on individuals and their decisions, conscience and activities, but on the other side, the media are the institution least trusted to have any social weight (Rayuso, 2011). There are several reasons as to why the condition is like this. For instance, the media are seen to have a significant influence on individuals because of its ability to influence the actions of individuals and even children. There have been several of surveys that have pointed out the paramount influence media have had on the American culture. These influences range widely and they include the influence on lifestyle, clothing, style, manner of speaking, and violence when it comes to the younger generation (Baker, 2004).

Alternatively, the media are viewed as the most untrustworthy institution because of their ability and tendency to exaggerate, violate privacy and feed the society with unsuitable and at times inappropriate information. For instance, the media are blamed and untrusted when it comes to vulgar and sexual material usually aired on the television and witnessed in movies and films. Because of these two sides or faces of the media, that are not positive, the mass media today has to assume a number of responsibilities because it is much recognized and followed by a large percentage of the population (Baker, 2004).

The mass media, for instance, has to be careful on the kind of information they avail through TV programs, movie, films and information on magazines, internet and the newspaper as it influences the society largely. It is also the responsibility of the media to feed the society with all the relevant information that they need to live their lives comfortably. Hiding significant information or releasing only a part of it is not responsible. Another responsibility the media have is to inform individuals about elections, political leaders and parties and their visions. This way, the media have a huge influence on the decisions the society has during elections. The media does this through dispensing specific information to either favor a candidate or tarnish their reputation. Either way, the media play significant roles in the American politics today (Baker, 2004).

The media do not only have impacts on politics and society alone, they also significantly affect journalism and techniques and methods informed. With the emergence of advanced technology, journalism has taken a new course that does not just require the TV, radio or newspaper to spread information. Today’s America receives information from a wide range of channels most of which are online. As it follows, journalism has had to advance its methods of communication to remain at par with the rest of the world (Rose, 2010).

The above are only a handful of instances of how the current mass media affects and influences the American culture.

The Black Table is still There

In the book ‘The Black Table is still There’, Lawrence Otis Graham who is the author of the book and a graduate from the Harvard law school and Princeton University goes back to his mainly white junior high school to pay a visit. During his visit, Graham comes by something that he previously thought that he could see it again after so many years of being to the school as he had forgotten it during his adulthood days. In the cafeteria of his white primary school lay a peculiar lunch table, making Graham flashback to his olden days of childhood explaining why the black table affected him.

Graham begins the flashback by stating that most of the time that he was the only boy in most of the activities that he attended that included like in his high school tennis team, the easting club as well as the summer music camps. Despite attending all the gaming and competitions activities at his school, he was not the only black American in his school. Graham states that there was a scenario that has made him startled most during his school days. He narrates that the existed a black table during lunchtime, the black table was a group of African American teenagers that could come as a group sitting at one of the tables in the cafeteria. Graham narrates that he would never sit at that table no matter whom they walked into the cafeteria as he feared to lose his white friends by siting with the black individuals, as he claims that he could be making an anti-white racist statement.

Graham showed his resentment to the all-black lunch table and believed that the black kids were the reason as to why other children did not mix more. I tend to agree with him in this statement as people tend to view teenagers who separate themselves into individual groups as being anti-social people and that they don’t like mixing with other groups. Lawrence blames the black kids for being the barrier of integration in the school and all his little world. Graham, however, states that he was wrong as, during the same time, there were at least two tables of athletes that included an Italian table a Jewish boys table and a Jewish girls table. Where he usually sat, a table of kids who often were onto heavy metal music and a smoking pot, a table belonging to the middle-class Irish kids. Here, Grahams asks “weren’t this table just as segregationist as the black table?’ In other words, Graham realized that he was wrong, and he realized this because yes there existed an all-black table at his high school, but there was also a table where all the athletes could sit, all the Italians could sit, and the Jewish girls and boys could sit. The African American teens sitting in the all-black table were therefore not isolating themselves. They were imitating what everyone else was doing, sitting together with their kinds of friends and this concludes that Graham was wrong in his judgments and that he continues to admit that he was assuming the opposite of what was happening.

Graham would not be the only individual who could be associated with the wrong judgments as to some extents the inference is always correct. I think it is clear that we tend to separate ourselves and that it is natural that this still remains and exist in our society today even though we don’t realize it as it is done unintentionally. There are a few factors that try to explain why we tend to segregate ourselves. One of these reasons is that without our recognition, we tend to stay and associate with people to whom we own the same race, and country and this is because we feel that we tend to share the same interests and we feel that we go so much in common. The habit goes on every day without the people realizing it, happening in our schools and amongst our neighborhoods. It is not perplexing to find that some of the communities are divided into the same ethnicity.

We feel that just because we share the same culture and beliefs, we can relate with one another more easily and comfortably with people to whom we associate with than to those whom we don’t share anything in common. Just because someone else is of a different nationality and religion, we tend not to associate with them as much. This, however, does not have the implication that we can never be a diverse group of people but because of the force of the habit to not only reside with our nationality and religion but even in our daily activities and this is especially among the younger people in schools such as high school. The kids on a sports team or some other activities that tend to bring people together often is characterized by the habit of association with people from the same region and kind. The reason is therefore quite clear that all the associations tend to share some common interest proving Graham wrong as he comes to realize this issue much later.

Despite being in the association, racial segregation has been significant part of the current and the previous generations. Racial discrimination tends to have dominated in the past years where it was publicly known that being black or white meant that you could stick to your lanes. The issue of racial segregation was much evident in the American society, and this as well can give an inference of the origin of the all-black lunch table that existed in Graham’s high school. It can be evident that the blacks were exposed to some discrimination by the whites during lunchtime and that is the main reason that they decided to have a place of their own by assembling and sharing their moments together.

High schools and colleges are among the public institutions to which racism is common, and it wouldn’t come as a surprise that a certain group of people may prefer to stick together especially if they are the minority group. The reason as to why they will seclude themselves is to avoid unnecessary victimizations from the majority groups. To further clarify our doubts, we tend to see the Jewish and the Italians having separate tables as well alongside the all-black lunch table during athletics and other sporting activities. Majority of the people from other regions may not be at ease in socializing with people to whom they don’t relate especially in language and colour. Therefore it is very difficult to find a Jewish associating with a black at the first time while there are others of their kind. The trend has however been reducing, but still, the aspect of the black table amidst of the other tables has continued to be evidenced in the present generation. The main reason behind is due to the enhancement of the rich culture and traditions of the people. The individuals tend to carry it along with them; this is because for example during lunchtime a black person may not do the same practices like the whites and therefore may opt to associate with the blacks to whom they tend to share a similar culture.

Parenting Styles

Parenting styles refer to the combination of various strategies used by parents to raise the children. According to Diane Baumrind, there are four parenting styles to which parents are grouped, and this includes the authoritative parent, permissive or indulgent, the uninvolved or neglecting parent and the authoritarian parent (Steinberg, Laurence, and Nancy). The type of parenting affects the performance as well as the discipline of the children. It is important to adopt the best parenting style so that parents can influence a healthy growth and development of their children as they interact. Each of the parenting styles takes a different approach in raising children.

The authoritarian parent

The authoritarian parents have the perception that children should do as they are asked to and therefore should follow their parent’s rules without exception (Chao). These kind of parents are famous due to their saying the ‘Because I said so’ in the instances to which the child questions their parents the reason behind following the rule. The parents in this style aren’t open either are they interested in any negotiations as their main focus is the obedience of their children. The authoritarian parents do not allow their children to make decisions for any challenge or problem they face but instead, they make the rules and enforce the consequences with little regard to the opinion of their children. The parents in this category may use punishments to instill discipline to their children, and thus they invest in making the children feel sorry for their mistakes rather than teaching their children to make better choices.

Children who are brought up by the parents who adopt this style tend to follow the rules much on time as they are brought up by strict parents. The children brought up with this style of parenting have a higher risk of developing self-esteem problems because their opinions are not valued since they were young. They may also be aggressive or hostile as they spend much of their time focusing on anger to which they feel on their parents rather than thinking about their future. As a result of being brought up by strict parents, the children in this category end up becoming good liars to evade punishment from their parents.

Authoritative Parenting

The authoritative parents make rules for their parents, but they also consider the opinions of their children (Baumrind). The parents validate their children’s emotions while at the same time making it clear that they are in charge of everything despite providing a room for the children’s opinion. The authoritative parents invest most of their time and energy in instilling discipline to their children. The reason is to prevent behavioral problems before they arise. The parents using the authoritative style of parenting are considered to use positive discipline strategies to their children to reinforce a behavior. Among the positive discipline, plans include the praise and reward systems that act as a motivation to the children to maintain the required behavior.

According to research, the children who are brought up by parents who use the authoritative style of parenting are most likely to be responsible adults in the future and to whose the self-esteem is raised making them feel comfortable in the expression of their opinions. The children who are raised with the authoritative discipline are often happy and successful in future, they are also more likely to be better decision makers and evaluating the risks. The children are also expected to make good leaders as they possess the qualities of a good leader such as ethical decision makers, discipline, timekeepers and at the same time have a higher confidence and self-esteem level as they were encouraged by their parents to provide opinions.

Permissive Parenting

Permissive parents are lenient to their children and often avail during the cases of a serious problem (Watson). The parents are quite forgiving, and they might give privileges if the child begs for them. The permissive parents are seen to take the role of a being more of a friend than being a parent and encourage the children to inform them about their issues. Besides, the parents on this category don’t put much effort into discouraging lousy behavior or bad choices. Children brought up by parents who are permissive are more likely to struggle in academics, they also exhibit behavioral problems as they do not appreciate the presence of rules and authority. The children may suffer low esteem as well as report cases of sadness. The children are also at a higher risk of obtaining lifestyle diseases such as obesity as their parents don’t struggle to limit the kind of food being taken.

Neglecting or Uninvolved Parenting

The uninvolved parents have little knowledge about the deeds of their children as they are not concerned about the welfare of their children (Hildyard, Kathryn and David). In this type of parenting, there tend to be few rules for the children, and the children may receive less guidance from their parents, less nurturing as well as reduced parenting attention. The parents expect their children to raise themselves with little or no assistance coming from them. The parents do not devote much time or even their energy into meeting their children needs as they are always neglectful of their responsibilities although it is not always intentional as at times they lack the knowledge about child development. At times, they may be overwhelmed with other problems such as work and other responsibilities that separate them from their children only availing themselves at the times of availability of which it is rare. The children brought up by uninvolved parents are likely to have self-esteem issues, and they perform poorly in school. The kids are also prone to indiscipline as their parents have no time correcting theory behaviors.

Various parenting styles instill different personalities to the children during growth and development. Children who are given opportunities to express their opinions with their parents have higher chances of being disciplined in the sense that their parents instill correctional measures in the case of in case the child goes against the set rules. For the uninvolved parents, the children may suffer lack of discipline, and they may be exposed to lifestyle problems as their parents have no control over their lives. Therefore parenting styles play a vital role in the growth and development of children.

DateMalacidins New Soil Antibiotic Summary

The article describes the discovery of a new class of natural product antibiotics, a novel of calcium-dependent cyclic peptide natural antibiotic by the name malacidin. From the study, the experiments conducted indicate that the mode of action of malacidin is different from the other antibiotics that require calcium. Bacteria are constantly evolving becoming resistant to drugs making it difficult to treat people that are affected by bacterial infections necessitating the need for more research for the development of antibiotics. The new target for the researchers is the microorganisms that live in the soil.

To make their research a success, the researchers collected over 2000 soil samples and picked daptomycin as a guide antibiotic for reference. Daptomycin kills the cells through the aid of calcium to disrupt the bacterial cell walls. The research team then used the DNA information encoded for the production of the antibiotic daptomycin as their guide during the study of the genomes of antibiotics used by the microorganisms in the soil samples. It is during the research that they came across the new family of antibiotics, the malacidins that can fight off infections using the same mechanism as daptomycin to kill the cells that are through using calcium to disrupt the bacterial cell wall. Contrary to the other antibacterial agents, the calcium in the malacidin family does not cause leakage in the cell walls but instead disrupt the antibiotic behavior in different ways. The researchers tested the antibiotic sample on rats that were induced with MRSA skin infection to test the efficiency of the antibiotic. The malacidin is reported to kill the bacteria effectively. Besides, the researchers examined the bacterial resistance on malacidins to which tested negative according to the study.

The next experiment would be to test the new antibiotic with other animals such as pigs and rabbits with induced conditions before clinical trials with humans, and this will increase the efficacy of the antibiotic to humans to which would also turn positive. Besides more research should be conducted to find more members of the malacidins family.

Introduction

Organizations have been faced with the need to provide connectivity or network within the same premises or in offices that are in different geographical locations. Some have also had to set up networks to help connect the employees whose duties involve a lot of movement to the facilitate access of required data in the headquarters. This has been done by setting up Wide Area Networks and Local Area Networks in the organization. In order to fully understand how these networking systems are done, WAN and LAN technologies must be understood first since they are the two main technologies used to facilitate networking in most organizations and corporations.

Local Area Network refers to the connection or networking of several computers and devices in limited area such as a building. It involves connecting a server to the same network that connects all the users ‘computers. This provides the networked computers with a similar subnet on their IP address. It is this subnet that the user enables the users in that local area network to access the server and perform networked tasks. The server is connected to databases that store all the required files belonging to the company. The users are therefore able to access this information through the subnet availed at the user ends of the network.

Local Area Networks are normally owned, controlled and managed by a single person or organization. Ethernet and token ring are some of the widely used LAN technologies. These networks are generally secure and do not have high maintenance costs due to their locality (Hughes& Blair, 2006).

Wide Area Network

This is a collection of computers and network sources that are interlinked and connected over a wide geographical area. This may be between the headquarters of an organization and its remote officers or its clients in a different geographical location.

Operation of WAN

WAN operates in two ways. This network connection can be wired or effective. In a wired WAN connection, the company has to physically run cables between its offices in the different geographical locations to a central hub. The cables are then connected to a LAN. This kind of networking is very expensive and time consuming. The cost of buying and fixing of the cables between the different locations is very high.

The second option is the virtually operated WAN. This is an option that entails creation of a virtual private network by the company. The virtual networks are popularly known as VPNS. These VPNs give data when the user computer requests it. This is the kind of network that can be able to sever a organization with remote office located in a different locality.

The virtual Private networks use technologies such as X.25, Frame Relay, and ATM as well as for their connectivity (Hughes& Blair, 2006).

ATM technology is a connectivity strategy used in WAN that uses point to point connection and thus facilitating direct flow of data in that network. Frame Relay technology on the other hand is a protocol that is used to transfer data over fiber optic lines. Another technology used is the X.25 which is a standard suite protocol that is used for packet data switching purposes. This technology works on physical, data links and network layers that are usually ISO modeled (Hughes& Blair, 2006).

WAN connection is therefore the best way to connect the organization question to its remote offices.

Designing the WAN

In order to set up the WAN, the company will have to develop a Virtual Private Network(VPN).The VPN will have a main LAN at the organization’s headquarters and another LAN at the office in the remote location. Unlike in wired connection or in cases where WAN connection is facilitated through the use of cables, the WAN design for this organization will be run on a public network (internet).some WANS are connected by lease lines such as Integrated Digital Network or Optical Carrier-3.Even though the leased line operated WAN are reliable and secure, the cost of set up and maintenance is usually very high making the e use of the internet the better choice (Nasser.2000).

There are two types of VPNs that can be used namely Remote Access and Site to Site VPN. The Remote Aces VPN will be suitable for this organization as it will allow user to LAN connection. Remote-access VPNs will allow secure, encrypted connections between an organization`s private network and remote users aided by a third party internet service provider. The operation of this kind of VPN requires the company or organization set up a dial up network set up. The organization will provide internet dial up account to its workers both at the headquarters and at the remote offices suing an Internet Service Provider (ISP).After accessing the internet the telecommuter will the use the VPN software to access the organizations network hence connecting the two locations.

The plan for setting up the network is shown below

Receiving Data

When a user in the remote officers wants to access data stored or available in the headquarters, he or she will have to make sure that the computer or device in use is connected to the LAN in the remote office. This LAN is the one that is then connected to the LAN in the headquarters. The system operates by a simple process that is referred to as client-server communication. Here the user computer or device will ask the server for information from its file and the server delivers the requested information. The request forwarded determines the data that is delivered. When the data requested is in the central drive in the headquarters, the server in the remote area uses the organization’s VPN to access the file and returns feedback through the same channel.

Receiving of Data

The servers that receive requests from the end users will process the request and deliver the right information as soon as it locates the file. When the servers on the remotes side is requested to deliver data or files in the headquarter file drive, it uses the VPN connectivity to establish communication with the server on the other end. The server on the headquarters then locates the requested file and then returns feedback containing the requested data to the remote office server. It is the remote office server that will then provide or avail that file or information to the user that had requested for it.

The servers at both end, that is the headquarter server and remote office server function as search engines. These servers access the organizations databases in their respective LANs and retrieve the request. For this particular organization with several users both at the headquarters and in the remote location, the use of multiple servers will be prudent. This will enable the users to access the information that they require with ease. In addition to that, it will make the WAN connection fast and reliable.

Conclusion

In order to fully set up a working, secure and efficient WAN for the organization; VPN will have to be set up. This will be the used to access company data both from the headquarters and the remote offices. Public network will then be used to link the remote offices to the headquarter of the organization
